Edit: I've set up a basic one using a free website.
The form can be accessed at http://www.spation.co.uk/mote/unban/form.php for the time being. I'll embed it into spation.co.uk/mote/ some day. Complete forms are sent to a new email address I set up (mote@spation.co.uk) which I'm happy for use from MotE administrators, just contact me for passwords/ports/etc.

The ban appeal form is lovely, but it's far too easy to fill out. Something more complicated is needed that asks for irrelevant information.
