I think I am going to give this one last try, I have a few professional DVD cases on hand and I could just print and hand cut and measure them out and post pictures later. I may rip off some designs from you guy's already but also bring in my own.

It should be rated T now, blood and gore is part of the game at default.

i modified it a bit and this is what i came up with

it was inspired by the sims 3 case so i removed a few elements.
i also removed all refrences to esrb because they have not actually rated it

also i dont see how it could be rated M, esrb does not rate online interactions so it should be teen because of animated blood (it does not look like and animate like real blood really)

actually i should make my own case instead of just edit yours
