We were already dating at the time, so it was obvious we were going to prom together, but I put a flower corsage in her locker with a little note on it. Good times.

I never got why people hold asking someone to prom on the same level as a marriage proposal. This isn't necessarily pointed at you seeing as other people have asked people to prom through elaborate songs or fireworks. But seriously, why not "Wanna go to prom."
Maybe I just feel this way because my school has very low spirit and the ASB only makes like 1-2 dances a year which nobody attends, not including prom, which people go to get drunk and partay.

it means a lot to the girl, it shows that you actually care about them

I'm sure you could get a fortune cookie, pull the note out and replace it without damaging the cookie. The note normally sticks out a little bit anyway, although you'd probably have to arrange that with one of the waiters, idk

what's so complicated about asking a girl to prom? why are you being all awkward about it? why do you have to ask people "how to do it"?
