AC4 PS3 Additional Content Code Won't Work

When I received the AC4 game, I first went to the PSN store to redeem all of my codes.

I specifically entered the codes in this order:
"Unlock your Exlusive Content Now!": The Trove of Mystery Pack, Captain Kenway's Legacy Pack. (I guess this was the Target special thingy.)
"PS4 Digital Version AC4 for $10" It's the thing that lets you get AC4 for the PS4 for 10 as an upgrade.
"Uplay Multiplayer Passport" It's the passport.

Then I went to the "60 Minutes of Additional Gameplay" slip, and it said to put the code in the left side of the case into the PS4 for the access code. I did, but it went to the "PS4 Digital Version AC4 for $10" download thing, and when I said confirm, it said "This Access Code has already been used", all though I have never touched it.

There are only 3 download things (I downloaded all of them) proving I never got the extra 60 minutes exclusive to the PS3.

Was the first one the additional 60 minutes? Or is it broken? Or was the code used before?

The bad thing is, I am 13 and in NJ it is required by law to tell why a game is rated M. If I stand there and smile, hand him GTA V, and my mom/dad gives the guy their license, what am I going to do?
