When he says "mentally ill", I have an inkling that he means that he's autistic, which isn't completely different, but is different (by that I mean that he does have mental problems, but he's not, say, schizophrenic). You can be autistic and be completely aware of it, and I am  almost completely sure that with certain mental illnesses you can be aware of having it.
