Newer versions of SMF (2.x) do it like this:
avatar_[profile#]_[random#].[extension]
(block it like avatar_[profile#]_*.*, for 1.x: avatar_[profile#].*)

That way your avatar blocking antics won't get forgeted up just because they changed their avatar (though linked images still break)
but at the same time avatars will finally be instant and you won't have to delete your browser's cache
