I looked into this awhile back because this was one of my fears. It's not very common anymore.
There are a few different factors. One of them is that we know how to positively identify death better than we used to, so we won't put a functional human being into the grave, or if we do, they were in some kinda deep trance state or were heavily drugged to lower heartrate.
There's also the fact that a bunch of coffins are rigged up with systems so that the inhabitant can alert the outside world if they wake up underground.
I remember reading, though, that a bunch of graves were excavated and they found a lot of scratch marks on the insides. That kinda scared me.
