I've never been to one that did, and that's including restaurants in Exeter, Plymouth, Oxford, London, Paris and Rome.
I think having refills is the exception to the rule.

I've never been to any fast food place that didn't! not just mcdonalds
they just have the fountain machines in some publicly accessible place, so even if they didn't want you to get refills it'd be kinda hard to make sure nobody does

Weird.

Only places around here with free refills are Subway (with specific orders), and Pizza Hut (although this only started at mine about a year ago).
Other than that it's only a few non fast-food restaurants that do free refills on soft drinks.
