It's the wideness of the smile and the creases beside the mouth. Before humans, smiles didn't exist. The closest thing to it was a threatening grimace. When a smile is that stretched, it looks a bit less human and more animal, and therefore more like a threat than a reassurance.

It's that and the fact that his eyes don't look entirely happy.

Still I don't think he looks like a serial killer. Just looks like he's trying a bit too hard with his smile.
