Falling and lava deal no damage

Lava and falling no longer kill me. I think there is an add-on causing this problem. I really don't know..  is someone can help me troubleshoot, that would be great

if you think it's an addon, have you tried figuring out which addon it is?
what have you installed since it was last working?
does the problem persist without any addons enabled or any addons in the game?
