i use a chisel

the best plan is to try some and use whatever you like. that is to say, i don't actually know what is the best a ha ha ha ha
ms paint, photoshop, paint tool sai, paint.net, adobe flash, all those drawing programs off the top of my head. researching is cool too
don't limit your way of thinking in the art of artiness. if something doesn't work out figure out a new method or tool or whatever

somewhat inspirational speech aside i use flash and a mouse

a mouse

no idea how anybody functions with a mouse good job
I sure as hell can't

there are many tools of smoothness

sometimes it's a percentage of drawing, sometimes its a selector edit, and sometimes it's that paint cursor lag mentioned kinda like this thing

but there usually is a way to get smooth out lines via settings of most drawing tools i would think
