Looks to me like a mix of the TF2 Flamethrower and Minigun, missing a few parts.

If you want it to be a shotgun, I would add a real stock and move the (awesomely beveled) barrels closer together. And then you should probably add generic gun stuff like a handle, trigger, and clip.

What's with you and sculpting faces? Modelling faces works well too, especially if you have a good reference.
