High quality.  Such examples are RTB, JVS, basically anything Kaje has released, and ummm.....not much else besides the good old duplicator and fill can.

Edit: Scratch that.  Good add-ons come from good add-on makers.  I'd say: Trader, Ephi, Randy, Kaje, Zor, Nitramjt.  Most of the things released by these people are good, and most good things are released by these people.  By most I mean nearly every.

I would hope my one released add-on would fit the list, but if I put my name there I'd be pulling a Bushido.

Anything unique, done correctly that we don't have.

I want to slap the next person to make our newest automatic gun or one hit kill ubar powered rocket lazer with ubar patened ubarness.

If it adds content to the game or makes something better, I like it.  Things like Duplicator and JVS Doors.  They are useful, they add nice content to game, they help build a lot of stuff.  I can't live without duplicator, i really can't, it's the first thing I download when i reinstall.

A unique event, such as SetPlayerTransform.
A useful tool, like the fillcan, or duplicator.
A new feature for builds, such as doors.
A well designed weapon, like Ephi's shotgun.
A quality game modification, such as RTB.
A minigame feature, like TDM.

But, not any of these:
A crappy event, such as setPlayerName (although V11 prevents usage of this)
A useless tool, like... well... Not sure.
A new spammy building feature, that isn't even actually new, such as the preset brick pack.
A terrible weapon, like... well just see the Weapons topic in the drama section.
A crappy game modification, such as ANY public RP(G) mod.

A mod, weapon, item or anything that adds something new and original to the game, or does it in a unique way. For instance, the original Sawn-Off Shotgun: nobody else had used the 'spread fire' or 'multi fire' script before that. The Flak Cannon, adding multiple firing modes to a weapon.

Mods that are well-designed and fit with the concept of the game, in models and function. Mods that perform as if they had come with the game, using its own features - JVS Doors since v9 has been a very good example of this.

No console errors. Missing textures included. This effectively rules out every idiot's weapon mod ever.

A good ad-on is one that can be used for a number of things, a death match, racing, exploring, role-playing, or otherwise. It's also one this is done well and looks good at the same time :)
