It's a terrain file and you generally copy it from another map and then edit.

To get a terrain file just base your map off of one that already has one. There's no point in creating one from scratch.

Yes, you need a skybox, but you can use the ones that came with the game. The nice thing about using an existing map as a base for a new map is that it will already have a working skybox (and matching fog) set. You can keep it or change it.

If you plan on using water in your map, don't use Bedroom as a base. It's terrain block size has been changed which breaks water for some reason.
