Messed up decal

I made myself a decent face decal. I included the namecheck.txt, description.txt, and the face png file (one in thumbs and one in the original folder). So I put it all into a zip folder and moved it to addons. When I went to the player option, my face was there. But when I put it on, it was all stretched and completely not centered on the head. Can someone plz tell me what I did wrong? Thx in advance

So, the icon was there, but the face was jacked up? Were the icon and the actual face the same size? Because that might be yer problem.
