Unknown Action means they're doing something that actually hasn't been registered to show a message in the "Who's Online" page.
The "Nothing, or nothing you can see..." is a page that you cannot access, such as hidden boards, editing a specific users' profile (primarily their own), or some other thing I don't know about.
