Wounds and such can be scripted and made easily, but most modders are too busy or are too lazy to make it because it is too simple.

How it would work:
Force downloading for the wound decals.
Changing decal script with onCollision or something else.
(Might be a huge download if you make it recognize the player's current decal to set the wounds for that version.)

And for the blood thing, you use particle emitters that last a very short while and act like blood OR spawn a couple of "blood" datablocks and make them fall and eventually dissappear.

It wouldn't be very realistic because you'd be wearing say, a suit, then when you get attacked all of a sudden your suit's gone and you have a tiny cut on your left nipple.

That was the general idea, I thought.

The functions that set the decals replace the old one with the new one. I don't know whether it'd be possible to edit it to allow you to have more than one decal, I don't think it's possible to do that though.
