I could never figure out how to use engineer

I play Spy the most, although I need specific aid from the team or otherwise I just get steamrolled.

Oh haha, I just remembered an epic moment.
This engineer who was new kept using his wrench and kept hiting me when I was disguised.
I asked  him to stop or he'll die.
He says I'm wearing a "clown mask" and it means Im disguised as his team.
WHEN I WAS ON HIS TEAM
Then, the enemy team and came and shot him dead lol

1. Go to place.
2. Put down sentry.
3. Hit sentry with wrench.
4. ???
5. Profit

How do medics defend themselves?

If they HAVE to, they can use their syringe gun or bonesaw.
But having a decent healing target who defends you is more effective.
