as was already stated, he was taking advantage of something in a handicapped add-on.

Cca messed with this add on called "Autobridge" it has a back door that enables him control over your server somehow thats how hes "hacking"your servers.
So anyone who downloads it when he joins your server and you have autobridge he can take the role of auto admin and ban everyone.

lucky me, i was seriously considering downloading that addon but i didn't know exactly what it did so i decided not to.
