Jumped out of my helicopter in GTA IV but then it tilted to the side as it fell so the rotor blade killed me as I was helplessly on the ground.

I mined a iron block then fell into lava right below it but i dig out just in time before i died, None of my items got burned. This was on the minecraft adventure island map.
