Accidentally pressing Y when in a helicopter in GTA IV, falling towards the ocean, you hit ground right in front of the ocean where you would have survived.
FFF--

Worse: You and your friend are having fun with Helicopters out in the ocean. He falls in, and screams "Godforgetnigdammit!" because shore is so far away. You offer to try and pick him up by keeping the Helicopter JUST above the water (I've actually done this once or twice before). The Helicopter suddenly gets sucked into the water, taking out most of his health, and leaving you both stranded.
