All of them and there's nothing you can do about it!

Thumb on the leftmost mouse button, index finger on the left mouse button, middle finger on the scroll wheel, ring finger on the right mouse button and little finger on the rightmost button.

What the forget? My hand has to arch above the the mouse just to get it set up like that. And I'ts akward.

My middle finger for the wheel and right-click, and my index for left-click. My mouse  exclusively gets my right hand, though, since I only type with my left. :o

Middle only.

Why would you use index? What do you use to press the mouse button?

You're lazy, exercise is important!

Really?
That interesting.
I can't type without my right hand. I was taught how to type in Junior school, and I've learnt it to the extent that my right hand always controls the right of my keyboard.
I even keep my fingers in the "safety/default" position.

Fingers on left hand are arranged (From little finger to index) on A, S, D, F. Occasionally, I keep little finger on Left Shift for easy shifting.
Fingers on the right hand are arranged (Index to little finger) on J, K, L, ;.
Occasionally that twists though, depending on how I'm sat, so it's J, I, P, [. That way, my little finger is close to press backspace.

Switch between middle and index.

I learned the same way as you. However, I first learned how to type on a manual typewriter when I was younger, so positioning was key. If you didn't strike the keys in a certain way, your letters were too light or lopsided. But then when I learned how to key on a keyboard, I just found it more comfortable with one hand on the keyboard and the other on the mouse.

Interesting, though. We both learned similarly in youth and then our techniques grew apart. :o

I scroll with my middle finger, mostly because it's a habit.
Probably because I play CS:S too much.
I use Q + 1-4 or 5.

I've got my index on the left button, and my middle on the side of the scroll wheel. So I can easily right click or scroll.

Also, my default left hand position is shift a s d. Too much wasd gaming.
