I keep my middle finger on the right mouse button.

When I need to scroll, I move my index finger onto the scroll wheel.

I use my remaining fingers to hold onto the mouse.

I don't think you can shoot your weapon right when you switch weapons in most games

I use my number keys anyway because my scroll wheel is so sensitive that it's counterproductive to try to precisely scroll to the right weapon
