"Warhammer" refers to the fantasy, medieval Warhammer. Many of the same things apply, but the key differences I know are Chaos only has 3 gods and everything is medieval.

"Warhammer 40,000", or Warhammer 40k, or just 40k, is the futuristic grim future of the galaxy where there is only war.

Forget 40k, this is in maybe the 5000th millenium, and mankind still uses mainly swords and knights. There is daemons and orcs and chaos and such ravaging Earth, and humans do not have bolters to combat them with, only swords and axes.
