You can't kill people, not without causing major problems for yourself. The game's realism is brought to a new level - killing an innocent person applies to a record and makes you more notorious, which means more likelihood for sudden, unexpected police raids. By the end of the game, unchecked, you could be under constant police attack.

I was making up stuff for the sake of... Well, think of it like this: You fall into the ocean in GTA IV, you're basically screwed. Realism. I was being sarcastic, to imply that such drastic realisms be corrected.

If you fall into the ocean in GTA4 it'll take a long ass time to swim back to shore mostly but it's not impossible.
