Lindsay (Ashley C. Williams) and Jenny (Ashlynn Yennie), two American tourists in Germany, are drugged and involuntarily detained by crazed surgeon Dr. Josef Heiter (Dieter Laser) when they seek help at his house after their car breaks down. The women awake in a makeshift medical ward. They witness Heiter kill a kidnapped truck driver (Rene de Wit) after Heiter informs him he is "not a match". When the women wake up a second time, Heiter has secured a new male captive, Japanese tourist Katsuro (Akihiro Kitamura). The doctor explains that he is a world-renowned expert at separating Siamese twins, but dreams of making new creatures by sewing people together. He describes in detail how he will surgically connect his three victims mouth-to-star fish, so that they share a single digestive system.After Lindsay tries to escape and fails, Heiter explains to her that he had previously experimented with creating what he called a "three-dog", also joined mouth-to-star fish. However, the three-dog died shortly after surgery. Heiter tells Lindsay that the middle dog of his creation experienced the most pain, and as punishment for her escape attempt she will become the middle part of his centipede. Heiter performs the surgery on his victims, placing Katsuro at the front, Lindsay in the middle and Jenny at the rear. During his procedure, Heiter severs the ligaments in his victims' knees so they cannot stand and are forced to crawl, and sticks the victims together in a line with their mouths surgically attached to the star fish of the person in front of them.Once the operation is complete, Heiter attempts to train his centipede as a pet. During one such session, he watches with great delight as Lindsay is forced to swallow Katsuro's excrement. However, he eventually becomes irritated after being kept awake by the constant screaming of Katsuro (who, as the front part of the centipede, has his mouth free and is still able to speak) and the realization that Jenny is dying from blood poisoning. When two detectives, Kranz (Andreas Leupold) and Voller (Peter Blankenstein), visit the house to investigate the disappearance of tourists, Heiter decides to add them as replacements for Jenny in a new creation, a quadruped. Heiter succeeds in drugging one of the detectives, Voller, as the pair later leave the house to obtain a search warrant. The victims attempt to escape from the ward, crawling up the stairs, as Katsuro also attacks Heiter in the process. Their attempt to escape ultimately fails. Katsuro confesses to the doctor, in Japanese, that he deserves his fate because he had treated his family poorly. Katsuro then commits Self Delete by slitting his throat with a shard of broken glass.The detectives return to the house and forcefully conduct separate searches as an injured Heiter hides near his swimming pool. Kranz finds the makeshift ward along with Heiter's victims in another room. Meanwhile Voller begins to feel ill from the earlier drugging, and is later shot by Heiter. Kranz discovers Voller dead in the swimming pool. Heiter shoots Kranz in the stomach, and Kranz responds by shooting Heiter in the head. Kranz then falls in the pool, dead. Back in the house, Jenny and Lindsay hold hands as Jenny dies from her infection. Lindsay sobs as she is left alone in the house, trapped between her deceased fellow captives.
The film opens with the final moments of The Human Centipede (First Sequence), including the closing credits, playing on a laptop computer in a parking garage toll booth. A man, Martin Lomax, is watching the film. Martin is short, asthmatic, overweight, mentally ill, and middle-aged.In a metanarrative device, Martin is obsessed with The Human Centipede (First Sequence), watching it repeatedly at home and in his toll booth. He keeps a centipede in a glass terrarium at his mother's house, where he lives, and maintains a detailed scrapbook of the film, including sketches of the '100% medically accurate' procedures on joining the 'segments'. Martin is intent on recreating the fictional experiment of connecting people mouth-to-star fish, as portrayed in The Human Centipede (First Sequence), but desires to create a "full sequence" of 12 connected people, instead of the original film's three people. Martin's mother (who is emotionally abusive and homicidal towards Martin, as well as suicidal) brings this to the attention of Dr. Sebring, Martin's doctor, who suggests that Martin is using the centipede as a connection to being loveually abused repeatedly by his father, who is now in prison for molesting Martin. During dinner, Martin's upstairs neighbors play Hard Dance music at high level. Martin's mother pounds on the ceiling with a broom in an effort to quiet the music, which causes the neighbor to come down and begin first verbally, then physically assaulting Martin.Martin begins setting his plan in motion to create his 'centipede', acquiring a dingy, dimly-lit warehouse after killing the man who is leasing it. He leaves the man's car parked outside the warehouse, and begins abducting the 'segments' from the car park, knocking them unconscious with a crowbar, then binding their hands and feet with tape and driving them to the warehouse. His victims include:- An aggressive young man and his girlfriend. Martin notices them locked out of their vehicle, which they took without permission from the man's father. As Martin approaches them, the man becomes confrontational, and both are knocked unconscious by Martin. As Martin opens the doors to his van to load the couple, a third man is seen already bound and gagged in the back.- An angry man complaining about an ATM at the car park being out of service- Two drunk female club-hoppers, who catch Martin masturbating to (First Sequence) in his booth with a piece of sandpaper- A man and his pregnant wife. While abducting the couple, Martin discovers the man and wife have their toddler son with them, and places the crying child back in the victims' car.- Martin's abusive upstairs neighbor. When Martin's mother discovers and destroys his scrapbook, Martin takes his revenge by first forcing her to be bitten in the face by his pet centipede, then bludgeoning her to death with a crowbar. Martin then props her dead body up at the kitchen table. and then bangs on the ceiling with a broom to antagonize the neighbor to come down. When the neighbor arrives, Martin bludgeons the neighbor unconscious. Martin then assembles a potpourri of kitchen gadgets, woodworking tools, and assorted household items from his mother's house, puts them in a suitcase- Dr. Sebring's driver and a prostitute. After killing his mother and abducting his neighbor, Martin returns to the car park, where he witnesses Dr. Sebring and his driver being serviced in their car by a prostitute. Martin kills Dr. Sebring and abducts the driver and the prostitute.- One of the three actors from the first movie, Ashlynn Yennie (portraying herself). Off-screen, Martin has called the three actors' agents pretending to be a casting agent for a Quentin Tarantino movie, and tries to lure them to London. Only Ashlynn agrees to the film.Once Martin has his twelve victims, he begins assembling his 'centipede'. Martin severs the tendons in each person's knees to prevent them from fleeing. He uses a hammer to knock out their teeth one by one, fishing out their teeth with his fingers so they will not swallow and choke on them in a Self Delete attempt. He slices open the buttocks of the victims, and then uses a staple gun to attach each person's face to the next person's buttocks and star fish. Martin chooses Ashlynn to be the front of the "human centipede", so she does not have to eat anyone else's feces. During the assembly process, the pregnant woman is presumed dead, and another victim dies during the procedure, so the centipede can only be made with ten people, instead of twelve. This setback saddens and infuriates Martin.After assembly of the victims, Martin, now clad only in his underwear, begins experimenting with his "human centipede". After forcing his victims to crawl around him in a circle, Martin offers food to Ashlynn in a dog bowl, who refuses it and throws the bowl. Angered by this, Martin then force-feeds Ashlynn soup using a funnel and a length of tubing. His experimenting is interrupted by a voicemail from the agent of the other two cast members of "Human Centipede: First Sequence", expressing interest in Martin's 'movie'. However, the voicemail is overshadowed by Ashlynn screaming for help. To prevent Ashlynn from screaming again, Martin tears her tongue out with a pair of pliers. He begins massaging the abdomens of each victim, hoping to force them to defecate into each others mouths, but to no avail. Martin then injects each victim with a syringe of laxative, forcing each individual in the chain to explosively evacuate their bowels into the mouth of the person behind them. Martin breaks down in tears of joy as he watches his 'creation' perform.As Martin celebrates, the pregnant woman, believed dead, regains consciousness and runs outside screaming, visibly in labor and gushing blood from her uterus. She leaps into a car, locks the doors, and begins trying to start the engine. Martin chases after her and throws himself against the car's windows, unable to get in. At the same time, the pregnant woman births her child into the foot well. As the engine starts, she throws the vehicle into reverse and stomps on the accelerator, crushing the baby's skull under the pedal. Martin is shown crying in rage as the woman escapes. Meanwhile, one of the victims rips his face from the person in front of him, separating the 'centipede' into two halves. Martin discovers this, and begins shooting all the victims in rage. Ashlynn throws Martin's centipede cage at him, smashing the glass and setting the centipede free. Martin shoots and kills all the victims of one 'half', then begins shooting the others until his gun runs out of bullets. Martin grabs a knife and begins slicing the remaining victims' necks, saving Ashlynn for last. As Martin advances on Ashlynn, she punches him in the genitals, grabs the funnel Martin used to force-feed her earlier, shoves it into Martin's rectum, and drops his pet centipede into the funnel. Ashlynn is then stabbed in the neck by Martin, who staggers out screaming as Ashlynn slumps to the floor.The film ends with Martin watching the credits of the original film at work, while a child is heard crying in the background, leaving it ambiguous as to whether the events of the film were real or not.
