Gap on a door where the hinges are. My mom slammed the door and my finger got cut off. (I was a babby at the time and my finger got stitched back on.

Riding bike and some random drunk driver turns the corner when I'm crossing the street and barley misses my back tire.

A couple years back I was at a ski/snowboard resort in Alaska.
When I went up on the chair lift, my glove got caught on one of the bolts on the chair and the chair spun around, nearly dragging me off an enormous cliff with dead trees below. For some reason, the morons running the resort put the new employees on the beginner lift, so the guy took like five seconds to realize I was being dragged, and slammed the button just in time.
