The size of their brains make them incapable of being sentient or self aware. It does not know it is alive. The cat brain has a massive cerrebelum that manages its instincts and motor control. The key word here is instincts, which are responses to the environment biologically programmed into the cat's brain.

Humans do not have instincts, we contemplate our actions before we make them, even if it doesn't seem like it sometimes. We don't suddenly downshift into a reptilian state just because of surprise.

Even still, the pain we feel isn't controlled by instincts. When something hurts we tell our parents or go to a doctor. When something hurts a cat just scratches at it until it stops (probably because they evolved to considering the common parasites that are found on wild cats).
