There is also a way to start with less then 3 players.
Also a console command:
tf_mvm_players_to_start or something like that.
Not sure how it was, but there is a console command for that. ;)

playing medic on MvM is really sort of handicapped unless you get the share-canteens with healees upgrade, then you can make them invincible and CRITS
