It happened to me like, 2 seconds ago.

He's never hacked accounts. What happened with TomTheGeek was that I think he used the same password for everything, so through some trial and error, and maybe some guesswork - after Tom signed up on poiuyt's forum - he was able to sign into mostly every account Tom had that used the same password.

No idea what could have happened to everyone mentioned in the OP though.
