I think that's it except-
Good hosts. That's one I think we've been missing for a while. It seems like ever since Badspot made it so Port Forwarding wasn't needed- the floodgates just opened and any poor fool with an internet connection would host. Since anyone could host, the fear of making the host angry just became a joke. Why do I need to worry about being banned if I can just create my own server with a flashy title and get 10 players within an hour.
But that's just a guess.
On another note-
I used to host quite a bit. I used to spend hours at a time hosting so that I'd get- ooh 9-10 players at a time? I'd get repeat customers too. That was always fun. People came back, cause they liked my server and it was always up.
Since then you might say I've lost interest. The people who join my server just aren't very nice. They're usually rude- stupid- unknowledgable... y'know just- not fun to be around. And why would I keep hosting a server if it just keeps filling up with rude unintelligent people?
There are also a few other reasons I stopped as well. Such as age- school- other games- etc. But overall I'd have to say the main reason is just that there's no one worth hosting for anymore. Also spam. People come in- drop a weapon spawn and a baseplate, shoot a few people and leave and its just like- "hell man, clean up your stuff."
