Here's an incredibly simple one that requires the HE-Grenade to already be installed. It's sure to lag your server.

Just drop this .cs into your Add-Ons folder.

If you want one that will wipe the whole map or have a realistic mushroom cloud, this won't do it. But it's something I put together in a couple minutes the other day.
