How much harder is it than the regular difficulty?

It's not as bad as you think. It only increases the likelihood of champion enemies to appear. Most of the time, though, they're still uncommon.

It also makes bosses drop fewer hearts.

Still, It's not so bad when you get good enough at the game to no-damage the later floors.

One of my least favorite enemies. Doesn't help that they have a decent amount of health.

Some weird item combo made this happen, and i don't know what..

Before they wouldn't explode on impact, but shoot 4 bob bombs that split into 4 more bombs each.
