"drastic" i would have to say would be any special interest that can kill/harm someone. Otherwise it'd be too easy to call every person who has a special interest "mentally ill". I would consider a lot of my special interestes to be rather taboo first impression for a lot of friends i know. The opinion of drastic changes a lot depending on peoples comfort zone. So they'd probably think i was mentally forgeted up or something.

But when it comes to child enthusiasm its without question.

I don't think anyone with a special interest is mentally ill so long as they get consent with their partner before trying something, weird but not ill. However child enthusiasm, rape, gore, and cannibalism are mentally ill.
