Assuming you know how to do everything but add the ports, you can find your computer's internal IP by going to the start menu, then clicking run. Type in cmd and click enter. Type in ipconfig and some IPs should pop up, use the one that says IP-Address or something like that. Not dns server or dns suffix, just IP Address. That is the IP you should use so just type it in where you need it.
