They do nothing, and it is an issue. Hearing loss and tinnitus is common amongst veterans.

From some quick googling some do wear earplugs, but sometimes they don't even use them or have the time to put them on

Chances are if you are getting shot at one day and still live you will get shot again
The last thing you want to think about is "oh stuff maybe my hearing will have ringing" instead of "oh stuff i need to go to the hospital they shot my richard off" or "i cant hear at all now so if i get jumped again ill certainly die"

When I shot guns outside / indoors with ear protection (those headphones) I could hear people talking and such just fine
