The grate is too heavy to lift. You barricade the door instead with some nearby cabinets and tables, and then return to the area outside the bathrooms.
Then look for a way out
You're not sure which way is out. To the right, there is still a raging battle going on in the cafeteria. To the left, you just barricaded a door to prevent the guards from coming in, and you get the feeling that you've barricaded the only way out. Behind you is the hallway with the corpses and your cell. In front of you is the bathroom.

Draw gun
You take cover behind a corner, draw your revolver, and peek around at the barricade you just made.
"Jesus Christ! Those crazies barricaded the door!" A guard shouts as he tries to open the door in vain.
"I've still got that explosive that one of the guys tried to light! Permission to detonate it, sir?" A second voice shouts.
"Permission granted!" You hear and see an explosion, blowing the door open and sending the contents of your barricade flying. You quickly move your head back into cover to avoid being hit by a flying table. The guards start flooding in! Half a dozen of them, and they've got revolvers and shotguns!

Feign your death by jumping into a random pile of bodies.
You run back to the hallway with the corpses and pretend to be one of them! The guards run in front of the bathroom and slip on the massive pile of stuff. Disgusted, they run into the bathroom to wash themselves off, but they run into the inmate cannibalizing a doctor in the bathroom! The cannibal inmate rips and tears into the throat of a guard before being gunned down!

Two of the remaining five guards run into the hallway where you are feigning your death. One of them stays behind to check on the corpses, and the other one runs into your cell. The lone guard has his back turned! Easy kill!
Attack one of the Authorities and take their gun
You get up and grab the guard from behind, making sure to cover his mouth!
The guard lets out a muffled scream!
You stab the shard of glass into the side of the guard's throat. As the blood spurts out from his neck, the guard reaches for his wound in vain before collapsing to the floor in a heap!
The guard silently cries for his mother before expiring in a pool of his own blood.

Your Bloodlust level is now at Unnerving. You show only the slightest sign of your "sane and rational" facade cracking, which average people will not notice. Kills are still swift and efficient at this stage, but go another level higher...You already have a revolver, so you take the ammo from the dropped revolver and manually load it into your own revolver, giving it 5 out of 6 rounds to fire now. Seems that the guard spent most of his bullets before you got to him on a previous inmate.
As you finish getting more ammo for your revolver, the guard checking on your cell walks out and spots you... And the corpse of his friend.
"Hold it right there!" He whips out a shotgun and holds you at gunpoint.
