Cough syrup usually contains the chemical dextromorphan, and in high quantities can cause hallucinations.

Everyone's body reacts differently to chemicals, this is probably your case.

Also this isn't true, acetaminophen does not cause hallucinations.
In excess it cause ulcers and death.

I really wasn't sure what I was taking. For the sake of story telling, I just said a medication.
