I have all the milestones but I still need three pyro, nine medic, and eleven heavy.
For pyro I'm missing hotshot, pyromancer, and pyrptechnics.
For medic I'm missing Consultation, Peer Review, Big Pharma, You'll Feel a Little Prick, Blunt Trauma, Medical Breakthrough, Ubi concordia ibi victoria, Family Practice, and Blast Assist.
For heavy I'm missing Party Loyalty, Division of Labor, Crime and Punishment, Class Struggle, Krazy Ivan, Icing on the Cake, Kollectivization, Redistribution of Health, Gorky Parked, Don't Touch Sandvich, and Borscht Belt.

I got every achievement for every single game all at once. No kidding, I'm that epic.

I need two more pyro achievements until I have all of them, Pyromancer and that one where you ignite spies with a sapper on a friendly building. Pyromancer needs 80-70k and I need to ignite two more spies.

I never really try to get achievements, most of the time they just happen out of random coincidence.

Edit: Acept for the taunting ones, i got them because i like being a bastard to people.

Why don't you just give a link to your tf2 stats instead of telling people to ask MLD? This would also allow us to see if you have been using achievement servers or not.
Same goes for you WhoDa.

It pisses me off when people try to make themselves look good but not give proof.
