i would think you are only invisible while holding the watch

You use the watch item to cloak. Putting it away or switching weapons uncloaks you before you can fire them.

With the Cloak and Dagger, your cloak regenerates while standing still: If you run around on an empty cloak-meter you are partially visible but at all other times you are invisible to everyone else.

A minor thing to do with changing the colour of the standard watch to the Cloak and Dagger, and the Dead Ringer hasn't been finished yet.
