Also You can go above level 4, so that's a lie. Only the monk and favored soul require paying. You can only have 2 players ON EACH SERVER without paying. You repeated the same thing like 5 times, also. You can also gain points in game by completing quests, so no, you don't actually have to pay with real dollars if you play enough.

You can. Just do a quest given to you in Stormhold [The guy that gives it to you is in the tavern]. One of the reward options for the quest will be a token that raises your level cap to 8. As you progress there will be quests that offer these tokens to raise your level cap. The max level cap is 20.

The money you spend in the DDO store can be earned by free players, it just takes a lot of work, the easiest way is to work for each of the houses in the game (coin lords, house denith, the free lancers, ect.). when you gain ranks for these groups you get money to spend in the DDO store.

Why does that matter? Because all those dungeons, classes, races and other stuff can be bought there.
