I don't think combine are from Xen.  Actually, Xen is a refuge for the creatures running from the combine.

I don't think so. If they're running from the combine, how come there are Advisors telling the Combine what to do? Advisors are from Xen, don't see them hiding.

It triggered some sort of signal during the portal storm the Combine could pick up and then tapped into the portal to teleport to Earth.
