It's pretty much the retail version with gtaiv.exe related to Steam. So even if it does not work, you would have to replace the gtaiv.exe. No piracy here, I hope.

Yes. I'm using a trainer on the steam version of GTA4 and EfLC.

I mean modable as in adding new vehicles, changing the player model/its skin and probably a whole new game modification with a story. I don't want to buy GTA IV if it isn't modable :(

You can do that with Sjaak's Simple Native Trainer. Just use it to change the player model to gabe fat man, and then press M+4 to go ragdoll.
