They either can not find the path to the item, or the item was misplaced, which might have happened if another dwarf grabs it first.
Check to see if you have a solid path to your food storage.

See, I had gotten attacked by a group of Giant Badger Bears. So they barged in and tried killing everything, and I freaked out and opened the insane dwarf's room. It ran out and started like organizing the dwarves for defense, it was crazy stuff.

Then they went apestuff and killed them all in a matter of seconds. I'm scared of them

On my first playthrough a Badger bear started killing off my people. I probably could have survived afterwards but I ragequit that world.

They organized in such an odd way, it was creepy as hell. It's like...

The insane one ran out of his room and stopped like right in the middle of the central lobby. Then all the other dwarves like ran out, and they were all like spazzing out around them, then they ran towards the entrance and killed the badger bears

I was like ..... ;-; watching it the whole time
