id actually like to see lead poisioning in a game, like when you get shot in the leg with a 9mm pistol you can pretty much die in an hour if thats not fixed

Missions where you have to stay behind someone, and if you are too close you have to restart. (GTA4, Assassin's Creed)

Missions without checkpoints (Mostly GTA4)

Alternate pathways that all lead the same way, but never go straight there

dogs, not just some dogs, but packs of dogs. in every game i've ever played, you piss a dog off and suddenly 500 loving dogs are trying to eat your testicles.

Races that are required to continue the game.

Fast as forget reloading like when your guy thrusts the pistol magazine right into the pistol or how you drop the assault rifle magazine then instantly get a new magazine and put it in. And how ammo somehow carries over to your new magazines.

how it should work is like swat 4 with a little bit a tweaking.
Lets say you have 5 mags, each carries 30 bullets, you fire 5 shots off of mag 1 then you take it out, store it for later and then put another one in. now since mag 1 wasnt empty you have 31 bullets and mag 1 has 24 as it took the 1 round that was chambered into the next one. Whenever you reload it should automaticly put in the mag with the highest number of bullets in it. something like that

Good idea and yeah I liked Swat 4's ammo system.

Racing in non-racing games,
