Motherloving escort missions and motherloving timed missions.

Escort mission: herp derp there's an army between you and here and you need to defend this handicapped baboon of a person oh did i mention they run ahead of you kthxbai
Timed mission: oh hey our lazy ass engineers cant stabilize this engine before it explodes you need to get off the ship before boom oh did i mention the tough and annoying enemies LOL

the part of some Brotherhood quest in Fallout: New Vegas where you have 60 seconds to find a virus in three terminals
it was insane difficult

When games make you load every time you enter a house or new area (no matter the size)

Example: Fallout 3 and new Vegas

Not super annoying, but it can get on my nerves.

"we don't need to make the enemies better in combat further in the game, lets just raise their numbers"
Basically, it's the same handicapped enemy you fight on easier difficulties, but he has more hp and does more damage.

This doesn't make it challenging, it's just as boring if not more because you're pretty much doing the same thing but for a longer time.

This is why I like difficulty systems in dynasty warriors games. Instead of just making them stronger, they also make the AI guard, try to break your guard, escape combos, do combos, etc.

"hey guys lets reskin a game, add virtually nothing to gameplay, and sell it for 60 bucks! We can call it Call of Duty X"

I hate unfulfilled endings.  I beat Borderlands alone the other day, I don't care that the boss was easy to kill alone but the drop and loot that I got at the end were just so god damn useless.  They were way below my level and below my weapons that I had.

Play with no expansions and do the second playthrough.
It's the best Boarderlands experience you'll ever have.

HERP DERP SHOOT THESE FLYING ROCKS WITH A RAILGUN DID WE MENTION THE STORM LASTS FOREVER AND YOUR TURRET IS INCREDIBLY WEAK?
