The biggest mistake dead ringer spies make that I see ALL the time, even from good spies, is they disguise immediately after feigning. That makes a smoke could that's visible for a short time after their "death". It makes it so obvious, and can even hint at which direction they ran, even if the feign was timed well.

well there used to be a strategy for disguising as their (the spy's) own team, feigning, and then changing disguises to the enemy so that they won't be expecting to see a different person
of course that strategy was meant for doing behind walls and such but some never read that far

Strategy or not, it should be common sense to wait 2 seconds before disguising while feigned.
