It is also sometimes used specifically as an insult to gay people.
Then again, there are contexts where it is not (such as outside of the US, mostly).

In my experience, the majority of "cigarette/ friends" I've seen used weren't in a homophobic manner, but to essentially call someone lame. Same goes for bitch. It's all about the context. Words change their meanings all the time.
