Think about this:
You may not exercise but you work real hard at a job or something.
Or maybe you exercise but not work.

You're only lazy if you don't exercise or do work.

He probably implied being out of shape and lazy go hand-in-hand and as such made a sarcastic comment about a statistic that he saw.
