It clicks, makes grinding noises, hums, scratches something but it's still running.

If you're running Windows, run a CHKDSK on the drive. If it's your main drive, you'll need to reboot into Real Mode or Repair Mode. If it finds bad sectors, it will save it in the system registry and not touch that area of the drive.
